Respecting Your Rights |
The Federal Health Insurance Portability and Accountability Act addresses the use and disclosure of medical records and information. It relates to protected health information, marketing or fundraising activities, directories and notification of family and friends concerning health-related issues.
Our privacy notice describes how information about clients of The Richard Simon Center for Alzheimer's may be used and diclosed. Our intent is to comply with the law and make reasonable attempts to limit the unintentional release of protected health information. Please review it carefully.
Appropriate health information regarding a client's care via consultation, payment, other health care services, assessments and evaluations may be disclosed. You, or your legal representative, have the right to request how we restrict your protected health information.
We may use and disclose you information in the following situations:
- Maintain a directory with names and phone numbers
- Use communication tools at health care centers, wellness offices and administrative conferences
- Converse to communicate a client's needs to staff and providers
- Disclose general information to family and friends unless otherwise instructed in writing by the client and/or legal representative
- Release specific information to family and friends unless otherwise instructed in writing by the client
- As a non-profit entity, participate in fund raising activities
- Use videos, still photographs, or electronic images for advertising, trade or other promotional purposes
- Newsletters and newspaper articles. For special uses in advertising, trade or external promotional purposes, such as a brochure or commercial, an authorization shall be obtained from the client identifying the special use prior to their use
- Effectuate reimbursement on the behalf of the client including but limited to third party payer, federal, state and local government agencies
